Customer provides information

A fictional department store wants to learn about customer preferences in order to encourage customer loyalty. In return for providing this information the customer is rewarded with a discount voucher.

The voucher is coded with a unique barcode. When the voucher is presented management can associate the purchase information (goods bought, time, date etc) with the details provided on the web page form. This is just an example, a real system would collect more details.
